  • If your family already receives SNAP/TANF, please be sure to include your case number when it asks if your family receives supplemental help.
  • Case numbers usually start with the letter "C" and it is not your EBT card number, however, it can be found on your EBT card. If you only received P-EBT Benefits (benefits received on a white card) and you do not have a case with Social Services, you are NOT a SNAP/TANF recipient and this field should not be filled out.
